BOOK A DATE

The form below is my preferred booking option for all new clients. Submissions received on this form receive priority screening and responses. Email is more than welcome as long as you send me all the information requested on the form. Please ensure to submit a completed booking form along with screening information and I will respond shortly. I do not accept incomplete forms.

SCREENING

For screening, I accept a combination of at least 2 of the following:

• A Photo ID sent with either a:

  • Selfie

  • Credit card (just your NAME visible, please don’t show me anything else)

  • Utility bill (again, just your name)

• Business card with the following visible: your name, company, and email ending in the corporate domain

• LinkedIn or a link to your page on your company’s website or a link to a third-party article and site with your name and photo visible

• 2 provider references from within the past 12 months.

  • Please do not send me your P411 or TER IDs as I do not accept them for screening, nor do I have provider accounts on these sites. These are NOT valid provider references.
